Connie Lan has been a volunteer at the DAWN clinic since 2015, and is now the scheduling and risk management coordinator for the dental workgroup. She has been active in caring for the underserved since her role as a volunteer at St. Vincent de Paul’s free dental clinic in Phoenix, AZ. It has always been one of her priorities to have an active role in the community, and it is her honor to be able to better serve the Aurora community.
Hello my name is Lissette and I am from Tucson, Arizona. In undergrad, I was Dental Director of the University of Arizona Flying Samaritans chapter where we traveled to Mexico once a month to provide free dental care. I love working with the DAWN Clinic and Dental Lifeline Network because it’s mission and vision is what I am most passionate about and it continuously reinforces why I decided to be a part of this profession in the first place.

Nursing students are an integral part of treatment teams every Tuesday night at Dawn. We work interprofessionally with MD and PA students in assessing and gathering patient information including medical and social histories and medication reconcilliation. We perform basic and focused assessments and collaborate on presenting report and developing a care plan for patients with our interprofessional team member and preceptors. Additionally, our volunteers are present at the triage station where they assess vital signs on each patient. Nursing students play a large role in educating patients about current and new diagnoses and serve as advocates for patient centered care.
Pharmacy students will be present in the clinic every Tuesday night. First year students are able to shadow second, third, and forth year students but are unable to volunteer directly at clinic. Second year students work at triage with interprofessional teams to ensure medication reconcilation is preformed on all patients as well as administer immunizations to patients in need. Our third and fourth year students work with the interprofessional student teams and preceptors to provide medication recommendations and counseling for both acute and chronic disease states.

I am a 2nd year physical therapy student from Crested, Butte, CO. I am passionate about empowering people to be in charge of and advocates for their own health. I love the opportunity DAWN presents for us to educate those who might not otherwise recieve it to be in control of their own recovery and, in return, they educate us as we apply practical skills to the clinic. While providing and managing, I kept having ideas for how DAWN clinic could run even more efficiently and volunteered for the position PT workgroup leader to become more inolved. I think Mitch defined this position below, so we’ll see if our definitions match (haha). This position serves as thebridgebetwen PT specialty nights and DAWN clinic nights as we meet monthly with all the other workgroup leaders. We are also responsible formobilizingall the PT workgroups to meet the needs of the DAWN clinic and organize projects. The workgroup leader recruits and develops DAWN’s network of subspecialist providers and volunteers. The leader coordinates specialty care services and referrals, oversees all Subspecialty Care workgroups, and manages monthly specialty care clinic nights.

Gretchen is beginning her second and final year of CON/ school at CU Anschutz. Gretchen is a Colorado native and has a prior Bachelors degree from CU Boulder in Ecology and Evolutionary Biology and a minor in Spanish. Gretchen is currently working as an Advanced Care partner at UCH on the Medical Health Services Unit. Gretchen became interested in public health when she studied abroad in Panama, and lived in multiple rural communities with very limited access to health care. Gretchen has been an active member of the CON/ work group since November 2016. While she continues to volunteer with the CON/ work group she is also very excited to take on a leadership role at DAWN with the ophthalmology team.
Amanda is about to be a second year CON/ student at CU graduating in May of 2018. Born and raised in Chicago, IL Amanda received her first bachelors degree at the University of Wisconsin-Madison with a dual degree in Sociology and Gender & Womyn’s Studies. Amanda has worked as a EMT and CNA for the last couple years and is currently an Advanced Care Partner on UCH’s Burn Unit. With one of her focuses being in community health Amanda started working with DAWN in November of 2017 and is looking forward to helping DAWN grow and being able to reach and treat more patients.
Lauren Walden is a third year medical student at the University of Colorado School of Medicine. Before medical school, she studied biological engineering at Cornell University. After college, she worked in brain imaging and psychiatric research in New York. Now, she loves learning as much as possible in medical school and helping out with the DAWN clinic psychiatry specialty workgroup.
Taylor Davis is from South Florida and is currently a first year medical student at the University of Colorado School of Medicine. She graduated from the University of Florida with a degree in Psychology and a minor in Health Disparities in Society. 医学部に先立ち, she volunteered in a variety of settings including the Mobile Outreach Clinic, which was a bus that delivered healthcare to underserved communities. Lastly, she hopes to be able to help expand the services that DAWN provides to benefit the Aurora community.
Josten Overall is a first year medical student, hailing from the San Fransico Bay Area. Josten graduated from the University of California, Davis is 2016 with a degree in Neurobiology Physiology & Behavior. Before coming to Colorado, Josten helped families access resources aimed at alleviating the burden of their social determinants of health, through her role as a health advocate at an FQHC in Sacramento, CA. Aligned with her passion for pediatrics, Josten served as a recreational leader for three years at Discover Day Camp, in her hometown of Union City. She looks forward to connecting with the Aurora community as co-leader of the Pulmonology and Pediatric Care workgroups at DAWN.

Patient Coordination Division

The role of the Care Coordination workgroup at the DAWN Clinic is paramount as we ensure that patients understand the next steps in their care, receive follow-up care, and are able to connect with the appropriate referrals. As patientsfirst point of contact with the DAWN Clinic, Care Coordinators also check-in with patients about follow-up screening tests and appointments. A large part of our services involve connecting patients to housing, food, employment, insurance, tobacco cessation, alcohol cessation, etc. services in the community. By ensuring efficient acquisition and sharing of patient records, our workgroup establishes partnerships and referral systems with local community organizations.
Diabetes Education works to support Type 2 Diabetics, identified within the Aurora patient population, through our presence in the clinic, and education regarding nutrition, exercise, and medications outside of Clinic nights. We work closely with groups within the community to identify high risk pre-diabetic groups, working closely with ADA in providing access to Diabetes Prevention Programs (DPP) and free exercise courses.
The Reproductive Health Workgroups aims to provide the Aurora population with resources regarding Contraception, Family Planning Services, and OBGYN Specialty services through contacts and organizations already available within the community.

Scheduling has two important roles: 1) Managing the master schedule for each clinic night, そして 2) Onboarding and welcoming all new volunteers. All individual workgroup schedules go through Scheduling so that volunteers who attend clinic can be coordinated for shifts at DAWN. All new volunteers are welcomed and oriented initially through Scheduling as well, including community members, working professionals, and students. They recieve their volunteer paperwork/immunization forms and are guided through choosing workgroups.
Dai-An is a survivor of the MD program who likes horror movies, slow-moving animals, and making gym jokes. She drinks too much coffee. Her main areas of interest in medicine are psychiatry and primary care.
Renee is a 1st year PharmD student who loves to workout, read and go for long walks when not in school going crazy with everything! She drinks coffee every day! Her main area of interest is Clinical Pharmacy, specifically in Critical Care or Emergency Pharmacy.

Quality and Safety Division

Matthew Applegate is a medical student and will graduate in 2018. Prior to starting shool, he was a research coordinator at Denver Health in the Neurology division coordinating stroke and epilepsy clinical trials. Matt’s hobbies include computer building / rebuilding and web development. Matt joined the HIT workgroup and the DAWN Clinic in January 2015.
Responsible for administering Practice Fusion, DAWN Phone and Fax services, the dawnclinic.org website and email systems, and other technical issues for the clinic.

Tong Young Huang is a pharmacy student expected to graduate in 2018. His origins begin in Corona, California, studied at The University of Hong Kong, and graduated from University of California, Merced with a Bachelor’s of Science in Human Biology. His interests in pharmacy and technology brought him to the Health Information Technology team. He hopes to be the clinician that bridges pharmaceutical knowledge between patients and physicians. Also, he hopes to assist in the technology that binds the organization together.
Conduct research studies evaluating DAWN’s care and impact on patients and volunteers. Oversee all research projects within DAWN via the DAWN IRB. Provide consulting support to other workgroups on their projects.

Procurement Division

I am a pharmacy student graduating in the class of 2019. As Medication Access co-leader, my goal is to increase our patientsaccess to medications, improve patientsunderstanding of medications, and increase student’s opportunities to learn about and counsel on medications.
Sanjana Rao is originally from India, but grew up in Portland, オレゴン. She has been an active participant at the DAWN Clinic since 2015 serving as a volunteer, manager and workgroup co-leader. Her goal is to increase access to medications to the underserved population, while supporting the growth and development of future health care professionals.
